Accepting New Patients
Heartfelt Obstetrics and Gynecology is always accepting new patients to our wonderful office. We want you to feel welcome and prepared with our new patient process. If you are a new patient with Heartfelt Obstetrics and Gynecology, we ask that you fill out the proper New Patient forms below. This will help expedite the process and cut down on your wait time. Insured Patient Policy
For patients with insurance coverage, Heartfelt Obstetrics and Gynecology will as a courtesy, bill your insurance company for you. Please be advised that it is the insurance company that makes the final determination on eligibility and benefits. You are responsible for obtaining any referral and or preauthorization required by your insurance company. Failure to obtain this documentation may result in a lower reimbursement or denial in payment from the insurance company. Our billing office staff may verify coverage and benefit information; however, it is your responsibility to know what your insurance will cover and how benefits will be paid.
Copay and Deductible Policy
All copayments, co-insurance or deductibles are due at the time of service, whether a set copay dollar amount such as $15 or $20 or a deductible of 20% - 30% of charges. Please be prepared to make this payment at your time of service. Heartfelt Obstetrics and Gynecology accepts cash, credit, personal check and money orders or cashier’s check.
Private Pay or Uninsured Patient Policy
For patients without insurance coverage (private pay or self-pay), full payments are due at the time of service. Please contact Heartfelt Obstetrics and Gynecology’s business office at 541-205-6890 prior to your appointment, to make arrangements for a payment plan if you are unable to pay your entire bill at the time of service.
